1 Doctor and 4 Nurse visits, additional visits are chargeable. Tele Consultation incase Doctor at home visit is not permissible

Monthly doctor’s visit
Visit from nurse or care managers 4 times a month for an hour
Charting and monitoring blood pressure and sugar every week
Reordering and filling up medicines
6 once-a-month medical checkups (if paid annually)
